If your child has a food allergy or requires Sharonville UMC Preschool to monitor dietary restrictions or medical conditions, please call the office to schedule a meeting with the Director prior to school starting. All food allergy or medical condition paperwork must be on file prior to your child's first day of school.
Food Allergy or Medical Condition Information
This form is required when a student has a condition that requires one of the following:

monitoring the child for symptoms which require the staff to take action
-
ongoing administration of medication or medical foods
-
procedures which require staff training
-
avoiding specific foods, environmental conditions, or activities
